About the role

Ennoble Care, a mobile primary, palliative, and hospice care provider, is looking for a full-time Hospice RN Case Manager to serve patients in and around Easton, Pennsylvania. Working within an interdisciplinary care team, you'll manage a caseload of home hospice patients, guiding families through end-of-life care with skilled nursing assessments and compassionate support.

Responsibilities

  • Manage an assigned caseload of hospice patients across the Easton, PA territory, overseeing assessment, care planning, implementation, and evaluation throughout the nursing process
  • Document all visits and clinical notes accurately and promptly in the EMR
  • Coordinate with attending physicians, hospice colleagues, and partner agencies to keep every patient's care plan on track
  • Coach patients, families, and caregivers on best practices for physical, mental, and emotional comfort at each stage of care
  • Partner with clinical leadership to resolve caseload issues and support ongoing professional development
  • Participate in patient care conferences to review and discuss cases with the full care team
  • Supervise and document nursing assistant care visits on a biweekly basis, or as assigned
  • Step in as an on-call resource for patients and families during off-hours or when a colleague is unavailable
  • Provide support to families at the time of death and throughout bereavement
  • Attend hospice orientation, in-service trainings, and community outreach programs

Requirements

  • Active Pennsylvania Registered Nurse license
  • 1-2+ years of nursing experience, ideally including at least one year in hospice care
  • Valid driver's license, clean driving record, and reliable vehicle for year-round travel to patient homes
  • Strong assessment and documentation skills, with the ability to write clear, comprehensive clinical notes
  • Comfortable supporting patients and families through emotional and end-of-life situations
  • Able to work independently while collaborating closely with an interdisciplinary care team
