About the role
Habitat Health delivers PACE (Program of All-Inclusive Care for the Elderly) care that helps frail seniors stay in their communities. As an RN Care Manager, you own each participant's care from assessment through follow-up, working shoulder-to-shoulder with an interdisciplinary team. It is hands-on nursing for someone who likes both clinical depth and relationship-driven case management.
Responsibilities
- Complete face-to-face assessments across physical, psychosocial, and behavioral needs
- Coordinate each participant's care end-to-end and contribute in interdisciplinary team meetings
- Manage medications and clinical interventions, including wound care and IV therapy
- Triage and stabilize participants in the outpatient PACE setting
- Educate participants and caregivers on carrying out the care plan at home
Requirements
- Unencumbered California Registered Nurse (RN) license
- Current CPR/BLS certification, or obtainable within 30 days of hire
- 2–4 years of clinical RN experience with medically complex or elderly patients
- 1–2 years of case management experience and time working with frail or elderly adults
