About the role
Form Health, a national telemedicine weight-loss clinic, is hiring a Patient Care Coordinator to support patients, physicians, and dietitians. You'll be the primary point of contact for patients — triaging clinical messages, coordinating scheduling and labs, and helping with benefits, prescriptions, and billing questions. 100% remote, full-time, for a certified Medical Assistant who's organized and empathetic.
Responsibilities
- Act as the main link between patients and their care team, triaging and resolving clinical messages
- Coordinate scheduling, pre-visit labs, prescription logistics, and prior authorizations
- Guide new patients through the program and answer questions about care and coverage
- Support clinicians with medical-record collection and documentation
- Handle insurance, billing, and membership questions and relay patient feedback to other teams
Requirements
- Certified Medical Assistant (CMA/RMA/CCMA from AAMA, AMT, or NHA)
- At least 2 years in a fast-paced primary care, obesity medicine, or endocrinology practice
- Excellent customer-service and communication skills
- Comfort with Slack, Google Workspace, and custom telehealth tools
- Available Monday–Friday, 9–5 PST (12–8 EST); US work eligibility
