About the role
CareTalk Health is hiring Registered Nurses for its virtual care team to manage medically complex patients from home. You'll blend care management, care navigation, and Annual Wellness Visit chart prep across programs like CCM and RPM — building care plans, closing care gaps, and coordinating orders. Fully remote, full-time, on evening or overnight shifts, for an RN with compact multi-state licensure.
Responsibilities
- Prepare comprehensive AWV charts and review histories, meds, and screenings with patients over video
- Provide telephonic and portal-based care management for complex patients across CCM, RPM, RTM, and APCM
- Develop, monitor, and update individualized care plans using motivational interviewing
- Identify and close care gaps — labs, screenings, medications, follow-ups
- Maintain accurate, HIPAA-compliant records in EHR and telehealth platforms
Requirements
- Active, unrestricted RN license in a compact state (willing to add states; company-paid)
- 2–3+ years of clinical nursing experience
- Experience in telehealth/virtual care and one of CCM, RPM, or care management
- Strong clinical assessment, documentation, and independent remote-work skills
- Dedicated, private home workspace
What we offer
- $35 per hour, full-time W2, 40 hours/week
- Evening (2:30–11pm EST) or overnight (11pm–7:30am EST) shifts
- Company-paid additional state licenses
