About the role
Abby Care is hiring a Licensed Practical Nurse in Atlanta to provide clinical oversight for pediatric patients receiving in-home care through Georgia's GAPP Medicaid program. The role centers on regular home visits to observe family caregivers, catch early signs of clinical change, and keep documentation audit-ready — all under the co-signature of a supervising RN.
Responsibilities
- Travel to patients' homes across the assigned Georgia region to conduct biweekly supervisory visits for children enrolled in the GAPP program
- Observe family caregivers performing personal care tasks and confirm the home environment is safe for the child
- Watch for changes in a patient's clinical condition and flag concerns promptly to the supervising RN
- Evaluate whether family caregivers are competently carrying out the care plan and coach them where needed
- Review and audit visit documentation to keep it compliant with GAPP and Georgia DCH standards
- Submit supervisory notes for RN co-signature within the program's required turnaround windows
Requirements
- Active, unencumbered LPN license in the state of Georgia
- 1–2 years of pediatric nursing experience
- Home health or prior GAPP program experience preferred
- Working knowledge of Georgia Department of Community Health (DCH) guidelines
- Strong, audit-ready documentation habits
- Reliable personal transportation for regional home visits
