About the role
Ennoble Care is hiring a full-time Hospice RN Case Manager to cover its Rockdale, DeKalb, and Newton County service area in Georgia. You'll own a hospice patient caseload end to end — assessing, planning, and adjusting care within an interdisciplinary team — while supporting patients and families through end-of-life care at home.
Responsibilities
- Manage an assigned hospice patient caseload through assessment, care planning, implementation, and ongoing evaluation
- Coordinate care with physicians, hospice staff, and community agencies to keep every patient's plan on track
- Document patient visits and care decisions accurately in the electronic medical record
- Participate in interdisciplinary team meetings to review patient status and adjust care plans
- Provide bereavement support to patients' families as part of the hospice care model
- Supervise and support certified nursing assistants on the care team
Requirements
- Active RN license in Georgia and graduation from an accredited nursing program
- At least 1 year of RN experience; hospice experience preferred
- Valid driver's license and an insured vehicle for home visits
- Current CPR certification
- Strong clinical assessment, documentation, and organizational skills
