About the role
Evergreen Nephrology, which partners with nephrologists on value-based kidney care, is hiring a fully remote Nurse Care Manager to manage a panel of kidney-disease patients across Chronic Complex Care, Compassionate Care, Post-Acute Care, Transitions of Care, and CKD Management programs. The role supports patients in the Central or Eastern time zone and requires working 8:30a–5p ET or CT.
Responsibilities
- Manage an assigned patient panel using nursing and value-based-care expertise
- Build trusting, empathetic relationships with patients and families for clinical and emotional support
- Act as an advocate and community liaison, helping patients navigate the health system
- Assess patient and caregiver needs, including social determinants of health, and translate them into care plans
- Coordinate the interdisciplinary care team to reduce fragmentation and manage kidney disease progression
- Monitor and adjust care management plans as needed
- Collaborate with nephrologists, APPs, and community providers on integrated care plans
- Support smooth transitions of care as patients move between care settings
- Educate patients and caregivers on treatment options
Requirements
- Associate degree in nursing
- Current RN license required; Compact License preferred
- Care management experience required
- Certified Case Manager credential preferred
- Intermediate MS Office / Outlook / Teams skills
- Home internet with minimum 25 Mbps download / 10 Mbps upload, hardwired recommended
- A dedicated, interruption-free workspace to protect patient PHI/HIPAA information
