About the role
Mission Healthcare — the largest home health and hospice provider in the western U.S. — is hiring a full-time Home Health Medical Social Worker (MSW) in Redding, CA, with a $5,000 hiring bonus. You'll conduct psychosocial assessments in patients' homes, build tailored intervention plans, and connect families with community resources across the care continuum. Pay is $45–$55/hour with strong benefits and mileage reimbursement.
Responsibilities
- Conduct psychosocial assessments of patients in the context of illness and environment
- Develop and implement tailored intervention plans from assessment findings
- Maintain accurate clinical records for social work services
- Provide information and referrals and act as a liaison to community agencies
Requirements
- Master's in Social Work from a CSWE-accredited program
- Active clinical Social Worker license (Board of Behavioral Sciences)
- 1+ year of healthcare experience (home health preferred)
- Current BLS/CPR, valid driver's license and reliable, insured transportation
