About the role
Ennoble Care, a mobile primary care, palliative care, and hospice provider, is hiring a remote LPN for a weekend-focused care coordination role. This is a phone-heavy administrative position — you'll build individualized care plans, keep patients on track between visits, and flag anything urgent to the clinical team, all without leaving home.
Responsibilities
- Build and update individualized patient care plans inside the company's EMR system
- Maintain regular phone contact with patients for care coordination and compliance check-ins
- Document every patient communication accurately and promptly
- Escalate patient emergencies or urgent concerns directly to the primary care clinical team
- Help patients' caregivers and families with scheduling and logistics
- Work across multiple software platforms throughout the shift
- Deliver consistently strong, patient-first customer service by phone
Requirements
- Active, unrestricted LPN license
- Comfortable spending most of the shift on the phone
- Strong organizational skills and ability to multitask across systems
- Available Saturday–Sunday 8:00am–4:30pm EST and Monday–Wednesday 9:30am–6:00pm EST
- Working knowledge of MS Office
- Must pass a drug screening and background check
- Authorized to work in the United States
