About the role
Included Health is hiring a telephonic Nurse Care Manager for its Care and Case Management team. You'll guide members through complex medical and behavioral health situations by phone or video, build care plans with them, and help them make use of their benefits. The role is fully remote and open to RNs who live in a Nurse Licensure Compact state, which includes Florida and Texas.
Responsibilities
- Deliver coordinated, patient-centered care management by telephone or video to improve member health outcomes
- Build care plans together with members and a multidisciplinary care team, and help members reach their goals
- Help members navigate complex medical conditions, treatment pathways, benefits, and the wider healthcare system
- Partner with members' local providers so care stays coordinated
- Provide compassionate, longitudinal follow-up and build supportive relationships over time
- Support members through acute episodes such as hospital and rehab stays with coordinated case management
- Line up clinical and social resources that address each member's needs
Requirements
- Bachelor of Science in Nursing (BSN)
- 5+ years of nursing experience, including 2+ years in care management
- Must live in a Nurse Licensure Compact (NLC) state and hold an active compact RN license in good standing
- Willingness to obtain and maintain licensure in multiple states
- Available to work until 6 PM local time (9-to-6 schedule)
- Comfortable discussing a wide range of medical conditions across all age groups
- Experience working remotely and using multiple computer and medical record systems
- Strong phone, clinical-history, and written communication skills; strict adherence to HIPAA
- Spanish speaking is desirable
