Remote nurse case managers coordinate care for health plan members and patients without seeing them in person. Most of the work happens by phone: assessing a member after a hospital stay, building a care plan, lining up services and following up until the plan is working. It is one of the largest job families on our remote nursing jobs hub, and it suits nurses who like the long-term, relationship side of nursing more than the bedside pace.
Every listing above is fully remote and was checked against the employer's own careers page. RN roles carry the case manager or care manager title; the LPN listings are care coordinator, clinical navigator and intake roles that support an RN team.
What remote nurse case managers do
- Telephonic assessments and care plans for members with chronic or complex conditions, revisited over weeks or months.
- Transitions of care: reaching patients soon after a hospital discharge to reconcile medications, book follow-up visits and head off a readmission.
- Specialty caseloads: current listings include maternity, workers' compensation and Medicare chronic care management.
- Coordination with physicians, social workers, pharmacists and community services so the plan actually happens.
- Documentation in the employer's care management system; some listings ask for Epic experience.
How much do remote nurse case managers make?
Across the 13 case management listings on JobSnag in October 2026, the midpoint of the posted pay range was about $41 an hour, roughly $85,000 a year. Posted ranges ran from $21 an hour for LPN care coordinators to $75 an hour at the top of one health plan's RN band, and every listing is full-time.
Some health plans post very wide bands (one runs from $54,095 to $155,538 a year), so read the bottom of the range as the likelier starting offer. For a national baseline, the Bureau of Labor Statistics puts the median for all registered nurses at $97,550 a year as of May 2025 (BLS).
Experience, licenses and the CCM
- Experience first. Listings commonly ask for two to five years of RN experience, ideally including case management or care coordination, and several prefer a BSN.
- The CCM is a plus, not a gate. The Certified Case Manager credential, awarded by the Commission for Case Manager Certification, shows up as preferred in several listings. You need case management work experience before you can sit for the exam: 12 months full-time under a CCM-certified supervisor, 24 months full-time without one, or 12 months supervising case managers.
- Bilingual helps. Several listings prefer Spanish speakers.
- License. Most listings require a multistate (compact) RN license, so nurses who live in a compact state have the widest choice. The compact license section of the remote nursing hub lists all 40 compact states.
Employers hiring remote case managers
Current listings come from health plans (CVS Health and Aetna, Elevance Health's CareBridge, Oscar Health), care-delivery companies (Included Health, Cadence, Accompany Health, Salvo Health, CareHarmony) and the claims administrator Sedgwick, which runs telephonic workers' compensation case management. Every Apply button leads to the employer's own site.
Case manager or utilization review nurse?
Case managers work with the patient, over weeks or months, to coordinate care. Utilization review nurses work with the chart, deciding whether a requested admission or service meets medical-necessity criteria. Choose case management if you want patient contact; choose utilization review if you prefer analysis and documentation.

Remote Nurse Case Manager Jobs — FAQ
Do remote nurse case managers need a CCM certification?
Not to get started. On JobSnag's current listings the CCM is listed as preferred rather than required. You also cannot sit for the exam until you have 12 to 24 months of case management experience, so most nurses earn it on the job.
Can LPNs do remote case management?
LPNs usually work alongside RN case managers as care coordinators, clinical navigators or intake coordinators rather than holding the case manager title. Four of the listings on this page are open to LPNs or LVNs.

Do I need a compact nursing license?
For most of these jobs, yes. Most current listings require a multistate license under the Nurse Licensure Compact, which you can hold only if your primary residence is in a compact state. A few listings accept a single-state license for a named state instead.
