About the role
Perry Health, a remote chronic-care company, is hiring contract Nurse Practitioners to lead diabetes education and management for Medicare patients. You'll run phone-based consults, evaluate diagnoses, meds, and labs, and prescribe CGMs where appropriate — all from home on a flexible 1099 schedule. Ideal for a multi-state-licensed NP with diabetes experience who wants to set their own hours.
Responsibilities
- Provide call-based care to members in Perry Health's remote patient monitoring program
- Evaluate medical history and data to determine program eligibility
- Identify and prescribe continuous glucose monitors (CGMs) for eligible members
- Serve as the primary clinical point of contact for escalations
- Collaborate with RNs, RDs, and coaches to reinforce clinical goals
Requirements
- MSN or DNP and active NP certification
- Unencumbered NP licensure in 3+ states (priority: AL, UT, IA, plus IL, IN, MI, GA, FL)
- 3+ years managing patients with diabetes
- Familiarity with CGMs and motivational interviewing a plus
- US-based with a quiet workspace and reliable internet; Spanish a plus
