About the role
Vivo Care is hiring a remote Care Navigator who holds an active Michigan RN or LPN/LVN license to support patients enrolled in remote patient monitoring and chronic care management programs. You'll track vital signs, lead wellness check-ins, and help patients stay engaged with their care plan — all without leaving your home office.
Responsibilities
- Review daily vital-sign data (blood pressure, weight, blood glucose) submitted through remote patient monitoring devices and flag readings that need follow-up
- Place monthly wellness calls to check in on symptoms, medication adherence, and general well-being
- Provide health education and coaching tailored to each patient's chronic conditions
- Monitor patient messaging and alerts, routing urgent concerns through the correct escalation pathway
- Document every interaction accurately in the care platform for the care team and the patient's chart
Requirements
- Active, unrestricted Michigan RN or LPN/LVN license (compact license a plus)
- Comfortable working 8:00am–5:00pm Eastern
- Experience with adult or geriatric patients, phone-based triage, or chronic care/remote patient monitoring programs preferred
- Familiarity with Medicare populations and motivational interviewing a plus
- Spanish fluency a plus
