About the role
UMass Memorial Health is hiring a full-time remote Ambulatory LPN for its Advanced Therapeutics program, delivering phone and video triage, medication education and follow-up for patients. The schedule is Monday–Friday, 8:00 AM–4:30 PM, and the union-represented role pays $28.14–$41.12/hour.
Responsibilities
- Run phone and video triage to assess symptoms, medication tolerance and clinical concerns
- Complete structured side-effect assessments and escalate findings to clinical staff
- Lead nurse visits on medication administration education, including injectables, and lifestyle assessments
- Educate patients on medications, side-effect management, lifestyle changes and program requirements
- Support pre-initiation workflows such as baseline assessments, lab coordination and pharmacy collaboration
- Monitor patients for safety labs, refill timing and insurance or authorization needs
- Answer patient messages within service-level timeframes and document accurately in the EMR
Requirements
- Current, active LPN license
- At least 2 years of clinical experience (ambulatory, primary care, specialty or telehealth preferred)
- Strong communication and patient education skills
- Ability to work independently in a remote environment
- Proficiency with telehealth technology
- Preferred: chronic disease management, triage experience, Epic familiarity
