About the role
Oscar Health's Complex Case Management team is hiring a Case Manager who works from home in one of 17 listed states, including Florida and Texas. You'll educate members, assess social needs, and connect them to community resources while coordinating care across settings. Candidates need an unrestricted RN license (state or compact) or an MSW.
Responsibilities
- Coordinate care across inpatient, outpatient, ER, and home care settings while protecting member confidentiality
- Partner with members and providers to work through complex health challenges and build personalized care plans
- Assess social determinants of health and connect members with community support resources
- Provide crisis intervention and advocate for members to remove barriers to care and treatment adherence
- Educate members by phone, secure message, or in person about conditions, hospital discharges, procedures, and new prescriptions
- Join process improvement work and new pilot programs as needed
Requirements
- Must live in AZ, FL, GA, IL, IA, KS, MI, MO, NE, NJ, NC, OH, PA, SC, TN, TX, or VA
- Active, unrestricted RN license (state or compact multistate) OR a Master of Social Work from a CSWE-accredited school
- 2+ years of clinical experience in hospital, outpatient, or community-based care management
- 2+ years working directly with patients who have chronic conditions such as diabetes, COPD, or hypertension
- 1+ year of care coordination and navigation experience
- Bonus: BSN, LMSW or LCSW, CCM or complex case management certification, Spanish fluency
- Occasional travel for team meetings and company events
