About the role
Salvo Health, a telehealth company focused on chronic gut and metabolic conditions, is hiring a remote LPN Care Coordinator. You will do regular patient outreach, support behavior change and health education, and work with dietitians and other clinicians through an app-based care model; a compact state license is required.
Responsibilities
- Provide care, disease management and health education to patients
- Support goal setting for patients asynchronously to help them manage chronic conditions
- Build personalized action plans and help patients create behavior-change plans
- Do regular outreach based on each patient's needs and preferences
- Coordinate with other clinical team members for a strong patient experience
- Create and share health education materials
- Work with a cross-functional product team to improve the in-app experience
Requirements
- Compact state license required (additional licensing may be needed)
- 2+ years of experience as an LPN or LVN
- 2+ years in patient-facing or customer-facing clinical roles
- Strong written and verbal communication skills
- Knowledge of medical terminology and general medical office procedures
- Comfort with digital tools such as Slack, Coda and Google Workspace
- Must be authorized to work in the United States
- Bonus: GI, weight management or GLP-1 experience; telehealth or startup background
