About the role
Cadence, a clinical AI company focused on chronic disease, is hiring a full-time remote LPN to work as a Clinical Navigator for Medicare patients. You will make outbound calls, coach patients on self-management, and document everything in Cadence's platform on a Monday–Friday schedule in Central, Mountain, Arizona or Pacific time.
Responsibilities
- Make outbound phone calls and give one-on-one coaching to patients with conditions such as type 2 diabetes, hypertension and cardiovascular disease
- Help patients carry out personalized care plans using motivational interviewing and SMART goals
- Assess health status, lifestyle, nutrition habits and readiness to change
- Teach medication adherence, symptom recognition and prevention under clinical supervision
- Track progress through regular check-ins and remote monitoring tools
- Document each interaction and connect patients with resources for social needs, transportation or medication access
- Work in real time with clinicians using the Cadence platform, Google Workspace and Slack
Requirements
- Active compact multi-state LPN/LVN license in your state of residence
- At least 5 years of clinical experience as an LPN
- Stable high-speed internet and remote work readiness
- Strong patient education, documentation and phone communication skills
- Comfort using AI-assisted clinical tools such as Gemini or OpenEvidence while keeping clinical judgment first
- Preferred: active California LVN license, Spanish fluency, chronic condition education experience
