About the role
CareHarmony is hiring a fully remote LPN/LVN intake coordinator who spends most of the shift on the phone, running preliminary health assessments for newly enrolled chronic care patients. The listing accepts any active multistate (compact) LPN/LVN license, so nurses in Texas, Florida and other compact states can apply; pay starts at $21/hour with potential up to $28/hour.
Responsibilities
- Take warm transfers from the Patient Enrollment team and complete preliminary health assessments for newly enrolled patients
- Answer patient questions and build an open dialogue about their needs
- Identify and coordinate community resources that benefit each patient
- Provide education and health-literacy coaching on chronic condition management
- Assist with medication management, including spotting concerns, adherence, and refill coordination
- Help ensure timely delivery of home health, DME, home infusion and other critical services
- Adapt quickly in a fast-paced, phone-heavy environment
Requirements
- Active compact/multi-state LPN/LVN license
- At least 3 years of direct patient-facing experience
- Proficiency with Microsoft Office
- Excellent written and verbal communication skills
- High-speed Wi-Fi and a home office or HIPAA-compliant workspace
- Comfort with extended phone work and real-time note-taking
- Nice to have: Epic experience, bilingual skills, additional single-state licenses
