About the role
CareHarmony, a Nashville value-based care company, is hiring a fully remote LPN/LVN to coordinate care for chronic care management patients. The job is a Monday–Friday daytime schedule with no weekends, and pay starts at $21/hour and can climb to $28/hour based on production, plus a quarterly bonus.
Responsibilities
- Manage a patient census and work to close gaps in clinical and non-clinical care
- Identify and connect patients with community resources that support their care
- Teach patients about managing chronic conditions and improve their health literacy
- Handle medication management: flag possible medication concerns, reconcile lists, track adherence, and coordinate refills
- Help arrange home health, DME, home infusion and other timely services
- Answer patient questions and keep an open dialogue to understand their needs
- Support referrals and appointment scheduling
Requirements
- Active multi-state (Nurse Licensure Compact) LPN/LVN license
- At least 3 years of direct patient-facing experience
- Working knowledge of Microsoft Office
- Strong written and verbal communication skills
- High-speed Wi-Fi and a home office or HIPAA-compliant workspace
- Ability to sit for long periods and spend extended time on the phone taking real-time notes
- Nice to have: Epic experience, bilingual skills, additional single-state LPN licenses
