About the role
Cadence is hiring a Care Management RN for its Advanced Primary Care Management (APCM) program. You'll watch patient data, step in with timely interventions, and support people with chronic conditions such as heart failure, hypertension, and type 2 diabetes. An active compact multistate RN license is required, and the schedule is weekdays in Pacific or Central hours.
Responsibilities
- Monitor patient vitals, symptoms, and clinical data to spot care gaps and patients who need timely outreach
- Develop and manage individualized care plans that follow physician recommendations and each patient's goals
- Talk with patients by phone and secure message to handle concerns and escalations with empathy
- Run virtual follow-up visits to onboard patients, adjust care plans, reinforce medication adherence, and encourage healthy habits
- Document every clinical interaction and prepare clear care summaries for primary care providers
- Work with interdisciplinary care teams on chronic conditions including CHF, hypertension, and type 2 diabetes
- Help refine clinical workflows so care management can scale
Requirements
- Active multistate (compact) RN license is required
- Associate Degree in Nursing
- Available Monday–Friday, 8:00 AM–5:00 PM Pacific or 10:00 AM–7:00 PM Central
- 5+ years of recent RN experience in an ICU or ER setting is preferred
- Experience in a chronic care management program is a plus; EMR experience (Athena, Epic) preferred
- Stable home wifi with strong upload and download speeds (a speed test is required before interviews)
- Strong clinical judgment, patient assessment skills, and written and verbal communication
- Comfortable working across tools such as Notion, Athena, Epic, Zendesk, and Google Workspace
